Drug List Information
Your
plan can choose to cover medications with either a preferred
drug list or a closed drug list.
For
your information and convenience, check out the The Preferred
Drug List of medications covered by many plan sponsors
that are considered appropriate from an effectiveness, safety,
and cost perspective.

Note: The CBCA Web site does not list all medications. It
includes only a representative listing of frequently prescribed
medications. If there are questions concerning the formulary
status of a medication, contact the CBCA Rx Member Services
help desk at 800-383-8737 for further assistance.
The
RX Claim Form should
be used when your pharmacy is unable to process a prescription
drug purchase and claim online, or when required to pay “out-of-pocket”
for a prescription covered by your plan.
